HISTORY OF THE DEPARTMENT OF INTERNAL DISEASES №2
The department of training a general practitioner in therapy No. 2 at the Samarkand State Medical Institute was organized in 1999 by Doctor of Medical Sciences, Professor Nazira Musaevna Khaitova, who has been leading it for 12 years. From 2013 to the present; the head of the department is Doctor of Medical Sciences, Associate Professor Eleonora Negmatovna Tashkenbaeva.
Over the 20 year period of time, the range of disciplines in internal diseases has expanded, in addition to the subjects of internal medicine, emergency conditions in the clinic of internal medicine, family medicine for the final courses of medical, medical and pedagogical faculties, in 2013 with the transition to a credit-modular system of education (the Department was renamed - internal diseases No 2), were added the subjects: the basis of internal medicine; therapy and clinical allergology for 3-4 year students of the Faculty of Dentistry; nursing in therapy for the 2nd year of the Faculty of Higher Nursing.
Since 2016, on the basis of the department, foreign students of 3-4 courses began to study in the subjects of internal medicine; emergency conditions in the clinic of internal medicine; family medicine. From the first days of the Department's organization, the clinical base is the Samarkand branch of the Republican scientific center for emergency medical care, in particular, the emergency therapy Department No. 2, the family polyclinic No. 2,3,4,7,8,12 of the city of Samarkand.
The scientific and medical-methodological activities of the Department of SamSMI are directly related to the scientific Department of the Samarkand branch of the RSCEMP. Since 2002, the assistant of the Department of GP No. 2 Tashkenbaeva E.N. headed the scientific division of the therapeutic Department. Research activities were aimed at further improving and improving the professional quality of the therapeutic service in the Samarkand branch, searching for optimal methods of treating patients in emergency conditions, establishing continuity of pre-hospital and hospital stages of emergency medical care. The initiated structural reorganization of the branch division contributed to the greatest integration of research work between the staff of the Department and the Department, which contributed to a significant increase in outgoing publications. To date, the Department of internal diseases No.2, together with employees of SBRSCEMA actively participates in scientific and practical conferences, seminars, symposiums, congresses of international and National scale. Over the 20 years of activity of the Department of Internal diseases No. 2 of the Samarkand State Medical Institute, and up to the present, a number of research works are being carried out to improve the methods of treating somatic patients in emergency conditions. The organizational forms, structure and content of the department's work at different stages of its development have changed, improved and integrated with the Samarkand branch.
Employees of the Department actively participate in medical work, research in the field of emergency medicine. With the non-direct participation of the Institute's scientists, the main stages of providing emergency pre-hospital and hospital care in the Samarkand region were organized, complex and intensive therapy methods were introduced, and the real possibilities of effective treatment for life-threatening conditions were expanded. Professors and associate professors of the department provide constant differential diagnostic assistance to patients with an unclear diagnosis.
More than 100 scientific works, including 2 monographs, were published under the leadership of the head of the department, professor N.M. Khaitova, who was also the deputy director for scientific issues of the SB RSCEMA. Employees of the Department made their scientific reports at many conferences and congresses of both National and international scale. N.M. Khaitova was the Vice-President of the Uzbek Association for reproductive health, a member of the editorial Board of the journal "Bulletin of the General practitioner", a member of the editorial Board of the magazine "Uzbekistan Medicine journal", was elected a Deputy of the Regional Council of people's deputies. Under the guidance of Professor N. M. Khaitova completed 4 candidates, 2 doctoral dissertations. During this period of activity, the department has graduated 8 clinical residents, 7 masters in internal medicine.
The department, together with the staff of the department, carried out research on the use of laser therapy for chronic bronchitis, bronchial asthma, myocardial infarction, hypertension; on the study of the immunological reactivity of the organism in order to expand the understanding of the etiology and pathogenesis of respiratory diseases and the development of tests that turned out to be useful in clinical practice.
Under the guidance of doctor of medical Sciences, Professor N. M. Khaitova from 2009 to 2011 scientific direction of the Department: "Development of new technologies for diagnostics, differentiated therapy, prevention and immunorehabilitation of patients with bronchopulmonary pathology". Members of the Department actively participate in the life of the Centre for advanced training of physicians SB RSCEMA regular workshops, clinical conferences, and master classes on actual problems of modern medicine.
Since 2013, the department has been headed by the former head of the emergency therapy department No. 2 of the SBRSCEMA, MD. Eleonora Negmatovna Tashkenbaeva, under her leadership 25 masters, 24 clinical residents in the specialty "Cardiology" of the Samarkand State Medical Institute and 20 clinical residents of the SF RSCEMP in the specialty "Internal Medicine" were graduated, 2 dissertations of Doctor of Philosophy of Medical Sciences (PhD) were defended.
From 2011 to 2014, research work was carried out on the applied program, PPI-10 "Improvement of public health protection through the development of new technologies and methods of diagnosis, treatment, prevention of diseases", under contract No A10-SS-01154, ADSS5, on the topic: "Development of methods for the correction of asymptomatic hyperuricemia to prevent the development of cardiovascular complications." According to the results of the study, were published 52 scientific works, a monograph on the topic: "Asymptomatic hyperuricemia: pathogenetic aspects, clinical picture, prediction and correction", publishing house "FAN" of the Academy of Sciences of the Republic of Uzbekistan, Tashkent -2014; textbook "Clinical pharmacology of medicinal products used in cardiovascular diseases"; Invention patent No. IAP 05346 "Method for the correction of endothelial dysfunction" was obtained.
Since 2015, the scientific and technical problem, the solution of which is aimed at the activities of the Department of Internal diseases No. 2 to the present, has the character of increased interest in the search for hereditary factors that predispose to the development of cardiovascular diseases. One of the main directions of scientific research is the study of the association of allelic polymorphism of candidate genes with the risk of developing coronary heart disease (IHD). Despite the huge amount of work on this topic carried out in leading genetic laboratories around the world, the data on the influence of certain genes on the development of coronary artery disease in most cases are ambiguous. The reason for this is ethnic differences; pleiotropic effect of genes; the presence of subclinical phenotypes; unaccounted for factors influencing the phenotypic manifestation of the individual's genetic characteristics, etc. For this purpose, the role of biochemical markers reflecting the development of endothelial dysfunction and oxidative stress in various clinical courses of coronary artery disease associated with UGU is studied, followed by the development of methods for molecular genetics prediction of unstable angina pectoris and criteria for the effectiveness of a personalized approach to their treatment.
Since 2018, the department of SamSMI and the scientific department of emergency therapy of the SBRSCEMA under the leadership of Doctor of Medical Sciences E.N. Tashkenbaeva works according to research work on the application program PPI-149 "Development of new types of treatment and methods of differential therapy and prevention of diseases, as well as the adverse effects of environmental factors, based on the identification of genetic polymorphism and other determinants of pathological conditions of the human body", under contract PZ-20170927280, on theme: "Development of molecular genetics methods for predicting unstable angina and a personalized approach to their therapy."
Employees of the Department are also currently conducting research in the following areas:
• Prediction of IHD exacerbations in the city of Samarkand based on the redistribution of the gravitational forces of the Solar System;
• Study of ways to correct endothelial dysfunction in patients with coronary heart disease;
• The study of the frequency of occurrence of dilated cardiomyopathy in conditions of emergency medical care;
• Study of the features of the clinical course, diagnosis, treatment and prognosis of early complications of myocardial infarction in an emergency hospital;
• Optimization of antiarrhythmic therapy in patients with acute myocardial infarction;
• Study of optimal approaches to the treatment of patients with acute coronary syndrome using modern methods of systemic thrombolysis, high-tech methods of diagnosis and treatment.
Up to now, more than 130 scientific publications have been published in these directions, including 4 scientific articles in the Scopus database, 1 textbook "Coronary artery disease - emergency conditions", 1 monograph "Molecular genetic aspects of coronary artery disease in asymptomatic hyperuricemia in Uzbekistan", 2 computer programs: "Personalized approach to the treatment of unstable variants of angina pectoris", "Prediction of IHD exacerbations in the city of Samarkand based on the redistribution of the gravitational forces of the Solar system (_Samarkand-I.exe)", depending on the genotype ". At the Department worked such outstanding teachers as PhD, associate Professor Kuyliev Murodillo Umarovich, who wrote a textbook for students "Internal diseases" in Uzbek, and also published 2 works of art: "Some Thoughts on Spirituality," "Teacher and Student.", PhD, associate Professor M. B. Makhmudov, who at the same time headed the Department of emergency therapy (1999-2005), PhD, R. A. Zhalilov, PhD, associate Professor L. F. Kazimova, PhD, senior lecturer Kh. a. Ashrapov, PhD, associate Professor D. K. Ishankulova, PhD Kh. I. Turdibekov, DMS.SH.KH. Ziyadullaev (2020, at the present time is Vice-rector for research and innovations of the SamSMI).
Employees of the Department have published 3 monographs, published more than 100 scientific and educational articles and theses in both local and leading foreign journals. Teachers of the Department constantly improve their knowledge and professional skills. They took advanced training courses in the USA, Estonia, Russia and other CIS countries.
Since 2014 she is the Chairman of the Association of therapists of the Samarkand region. Since may, 2020 Tashenbayeva E. N. is the editor-in-chief of the journal "Cardiorespiratory research", since 2014 she is the Chairman of the Association of therapists of the Samarkand region, by guanines professor Rizaev Jasur Alimjanovich.
SPIRITUAL AND EDUCATIONAL WORK OF THE DEPARTMENT OF INTERNAL DISEASES NO. 2
The spiritual and educational work of the teaching staff of the Department is aimed at implementing the leisure of our students in their free time, holding various cultural events. One of the main activities of the Department is the formation of high moral values among students, fostering interest in professional activities, the need for a healthy lifestyle, attracting students to cultural events of the Institute. To accomplish these tasks, experienced teachers of the department are attached to the curatorial groups, who are members of the curatorial council of the Samarkand State Medical Institute, whose activities are carried out according to the approved plan. Under the guidance of curators, the students of the faculty actively participate in public events, organize cultural leisure, and visit historical monuments and sights of Samarkand, theaters and museums.
Much attention is paid to cultural, physical education and sports. Acting, poetry, volunteering at the Call Center during a pandemic, training in sports clubs in various sports that function on the basis of the sports complex of the institute - this is not a complete list of what students of the Samarkand State Medical Institute do.
With experienced teachers, students develop their talents, achieve new successes, and take part in competitions of republican and international scale where they take pride of place. Also, students take part in the club of the cheerful and resourceful. For students, all conditions are organized for development both in the professional direction and in creative self-development and self-knowledge. As the Eastern wisdom says - "It is impossible to separate education from upbringing and vice versa, upbringing from education."
The spiritual and educational work of SamSMI is carried out on the basis of an approved plan, according to which various events are determined on historical dates, national holidays, sports competitions, etc.
Each curator has a work plan, on the basis of which various activities are carried out with the curator group. Each curator has information about each student, and his daily routine. There are also albums and discs that reflect the plots of the events. The dean and deputy deans of faculties regularly meet with group curators and students. Every Thursday, classes begin with an introduction to the "Information hour".

11. Employees of the Department carry out a lot of educational and methodological work. In 2016, the textbook "Clinical pharmacology of medicines used in cardiovascular diseases" was published in memory of Professor D. N. Ibadova; in 2020, the Department staff released textbooks in Uzbek, Latin graphics "Ischemic heart disease - emergencies", "Non-coronary heart disease" for 6th year students and masters in Cardiology, cardiologists.
Currently, the Department of internal diseases No. 2 employs teachers who simultaneously conduct research work for the degree of doctor of philosophy in medical Sciences (PhD), doctor of medical Sciences. The teaching staff of the Department, together with the therapeutic Department of the SB RSCEMA, headed by E. N. Tashenbayeva, regularly hold morning conferences, daily rounds and consultations of critically ill patients, consultations, calls are served by air ambulance in hospitals of the Samarkand region, conduct planned and emergency examinations of patients, consult patients assigned departments in the Center, participate and are opponents at clinical, pathoanatomic conferences. The number of published works over the past 5 years is 50 articles in journals, 150 thesis, 3 textbooks, 2 monographs, 10 methodological manuals and recommendations.
STUDENT SCIENTIFIC SOCIETY
The participation of students in the work of student scientific circles is an integral part of the educational process in the work of the department. More than 20 students, masters, and clinical residents are members of the Department's SSS. The materials of the scientific research work of the SSS are annually reported at the foreign, scientific practical conference of young scientists of SamSMI, and the best works are chooses to interuniversity scientific conferences This allows the department, upon graduation to graduate students, to orient themselves to the appointment of nominal prizez, to conduct a competitive selection for recommendations to make scientific research work, postgraduate studies abroad.
It should be noted that members of the SSS annually receive prizes at scientific and practical conferences, Olympiads. On May 15, 2020, resident of the magistracy Muinova Kamola was awarded a diploma of the II degree, 4-year student Nasirova Dilangiz was awarded in the nomination "Best Report" at the 74th International Scientific and Practical Conference of Medical Students and Young Scientists, dedicated to the 90th anniversary of SamSMI.
